Abstract
Laparoscopy is one of the most important surgical tools available to the gynecologist and is the surgical approach of choice for a variety of intra-abdominal problems. Knowledge and judgment are required to understand which conditions should and should not be approached laparoscopically. Careful technique will minimize, but not completely prevent, the risk of laparoscopic complications. It is critical to change the angle of insertion, the site of entry, and at times the entry technique according to the body habitus and previous surgical history. The major advantage of laparoscopy is decreased discomfort and disability for the patient. In many cases, cost is decreased as well. Major disadvantages of laparoscopy include an increased risk of bladder and ureter injury during hysterectomy and the uncertain effect on the prognosis of gynecologic malignancies. Robotic surgery and laparoendoscopic single-site surgeries are gaining ground; however, their long-term outcomes need to be substantiated.
