Photodynamic Therapy for Early and Advanced Lung Cancer

Abstract

Photodynamic therapy (PDT) is a minimally invasive therapy that can be applied in early and advanced lung cancer. It is based on the intravenous administration of a photosensitizing (PS) substance that accumulates selectively in tumor tissues, and when stimulated with a light of a given wavelength, it generates reactive singlet oxygen that mediates cellular cytotoxicity and ultimately results in cellular death. It is a minimally invasive treatment, easy to apply and well tolerated. Its main adverse effect is photosensitivity.

Current indications in lung cancer are limited to treatment of early “in situ” endobronchial carcinoma, roentgenographically occult, or synchronous primary carcinomas.

In advanced disease, it can be applied alone or in combination with other modalities to palliate tumor obstruction of the airway. Also, it can be indicated to increase operability or to limit the extent of lung resection in surgical candidates.
