Abstract

Effective care of older patients requires an understanding of the network of support services and locations of care available to those patients. Functional status often is the most important guide to care needs, and can be quickly and easily assessed in most health care sites. Patients and providers can choose from community-based support such as visiting nurses and home health aides to facility-based support in rehabilitation and nursing facilities. Support may be required for short-term or long-term needs.

A wide array of housing options for the elderly is rapidly evolving. These choices can range from strictly residential choices such as senior housing or boarding homes, to locations which offer some supportive care such as personal care, assisted living, or continuing care facilities. The level of support available can be an important consideration in successful discharge planning.
