Abstract

Flexible endoscopic interventions have historically been limited to the lumen of the gastrointestinal tract. The linear array echoendoscope, which enables real-time ultrasound visualization of a tool pushed out of the accessory channel, extends the reach of endoscopic intervention to targets outside of the gut wall. With the echoendoscope positioned within the gastrointestinal tract, endoscopic ultrasonography creates a platform to perform various transluminal interventions using coaxial tools already familiar to the flexible endoscopist. One of these tools is the self-expandable metal stent (SEMS). This chapter reviews recent applications of SEMS for transluminal therapy. These include pseudocyst drainage (cystenterostomy), gallbladder drainage (cholecystenterostomy), bile duct drainage (bilio-enterostomy), and gastrojejunostomy.
