Suture-Button Suspensionplasty for the Treatment of Thumb Carpometacarpal Joint Arthritis

Abstract

CMC arthritis is treated surgically with one of several techniques. Recently the trend has been towards simple trapeziectomy. Many of the other techniques available are considerably more involved and require a longer operative time as well as have a greater risk of additional complications. The current recommendation following simple trapeziectomy and hematoma distraction arthroplasty involves 4–5 weeks of immobilization with Kirscher wire (K-wire). This allows hematoma followed by scar tissue to form and the stabilization procedure to heal appropriately prior to K-wire removal and subsequent mobilization. Suture-button suspensionplasty using a suture-button device (Mini-Tightrope, Arthrex, Naples, FL) has been previously described. The long-term efficacy of this newer technique remains to be determined but the main benefit over the other techniques is the ability to begin early mobilization because this device is implanted and does not require any biologic healing prior to stabilization. This work describes this technique in detail.
