Abstract
This paper will deal with the patient related aspects of e-Health. The use of computing and networking has become an essential element in recent and future developments in health and care. Devices that lead to self-management, access to electronic records, decision support, social networks and social media are some of the major aspects. The production and availability of data and information will lead to a paradigm shift in the physician-patient relationship based on the addition of their respective experiences. To make this into a relationship of trust requires insight into the needs of both patients and caregivers. How do caregivers deal with possible knowledgeable patients; how can they assist to increase the patient’s knowledge; how will they (have to) react to patient record access. On the other hand, what will be the responsibility of the knowledgeable patient; what are his rights to be informed.

The Map of Medicine represents the state of the art in health 1.0. The map, intended for clinical use, is a web-based visual representation of evidence-based patient care journeys covering 28 medical specialties and 387 pathways.
