Abstract
This chapter describes open surgery through the right chest to gain access to the upper esophagus with resection of associated lymphatic tissue. The stomach is fully mobilized during the abdominal phase of the operation. With care to preserve the arterial supply and venous drainage, the stomach can be extended as far as the cervical esophagus.
