Intraoperative Evaluation of the Gallbladder

Abstract

Frozen section consultations on gallbladder specimens are most commonly requested for either (1) diagnosis of an incidentally discovered mass or mural thickening during routine surgery for cholecystitis, (2) assessment of bile duct or hepatic bed resection margins in patients with known gallbladder cancer, or (3) evaluation of liver, peritoneum, or omentum for metastases in patients undergoing surgery for gallbladder cancer. Most abnormalities that prompt intraoperative evaluation in the former situation prove to be benign, although they may mimic neoplasia at the time of frozen section. The purpose of this chapter is to discuss benign and malignant gallbladder tumors, as well as inflammatory mimics of carcinoma that may be encountered during intraoperative evaluation.
