Abstract
The exact format of the questionnaire as administered to subjects is given on pages 112–116, with one exception. When administering the questionnaire to subjects, omit the letters that identify the homogeneous item dimensions to which items belong and the signs that indicate direction of scoring of items. Both of these are given within parentheses preceding items.
