Abstract
Step 1: Problem list. Separate the complaints made initially by the patient from additional problems that are reported subsequently. Also include statement of difficulties the observer can recognize but the patient does not complain of directly, either because of avoidance or lack of knowledge and self-observation. Try to list, as well, the state of these problems after the conclusion of therapy. A descriptive (signs and symptoms) diagnostic formulation may be made here or in the next step.
