Laser Surgery of the Vagina, Vulva, and Extragenital Areas

Carbon dioxide laser therapy has clearly demonstrated its superiority when compared to other conventional methods of therapy for treating vaginal lesions. Lesions in the vagina, which were often overlooked by the gynecologist, are now receiving the attention they warrant. The vagina, with its many rugae, makes close scrutiny a task for the colposcope; however, careful manipulation and spreading of the rugae with a vaginal speculum allow for an easier and more thorough examination. Since the cervix and upper 80% of the vagina originate from the mullerian ducts, lesions affecting the cervix may also appear in the vaginal vault. Conventional surgical methods used for treatment of such lesions have proved to be difficult and less effective because of the limited operating space and obscured areas. The rigidity of the scalpel hinders the surgeon’s technique and often produces unwanted effects. Laser treatment has drastically reduced the incidence of complications associated with conventional surgical methods in this complex surgical field (i.e., urethral vesical or rectovaginal fistulae, hematomas, hemorrhage, sloughing of grafts, and/or stenosis with impaired sexual activity).
