Abstract

The 1980’s have witnessed a revolution in the approach to the treatment of patients with acute myocardial infarction. The publication by De Wood et al. (1) demonstrated the presence of obstructive coronary thrombi in 90% of patients who underwent coronary angiography within 4 hours of the onset of symptoms of acute myocardial infarction. The documentation of coronary thrombosis as a pivotal event in the pathophysiology of acute myocardial infarction provided the rationale for investigating pharmacologic and mechanical means of recanalizing acutely occluded coronary arteries. Subsequent studies documented that coronary arteries occluded by thrombus can be re-opened by thrombolytic agents or coronary angioplasty (2). Recent multicenter clinical trials showed that hospital mortality is reduced by the early administration of streptokinase to patients with acute myocardial infarction (3, 4). Thus, thrombolytic therapy is gaining increasing acceptance as the standard of care for the treatment of patients suspected of acute myocardial infarction.
