Abstract

Once kidney function replacement has been started, dietary therapy must be completely changed when compared with that for patients on conservative treatment. Patients on maintenance hemodialysis (MHD) have to face new problems related to oliguria-anuria (which appear quite early) loss of AAs with dialysate, poor removal of Pi, the catabolic effect of hemodialysis itself, and all the other side effects of hemodialysis, which are probably connected to some extent with the poor biocompatibility of the extracorporeal compartment materials. For patients on continuous ambulatory peritoneal dialysis (CAPD), there are additional problems related to the absorption of glucose from dialysate, the not uncommon complication of peritonitis, and protein loss that may reach very high values when this complication appears.
