Proteolytic Activities in Bronchoalveolar Lavage Fluid Correlate to Stage and Course of Interstitial Lung Disease

Abstract

In the clinican’s view interstitial lung diseases are a very inhomogeneous group of disorders, and in the last few years cell biologists showed us the reasons why. In exogen allergic alveolitis (EAA) or sarcoidosis stage I (SA-I) there is a predominantly benign course of disease, and one may find a lmphocytemacrophage alveolitis in bronchoalveolar lavage fluid (BAL-F). Advanced stages of EAA or sarcoidosis stage II & III (SA-II & III) are characterized by an increase of polymorphonuclear neutrophils (PMN). More often idiopathic pulmonary fibrosis (IPF) shows a progressive course, and in BAL-F we find a neutrophil alveolitis with PMN, eosinophils and mastcells as a sign of bad prognosis1. But if only differential cell counts in BAL-F are considered, lavage data show no good correlation to stage or course of diseases. This is due to the different kind of cell populations and cell interactions. Many investigators typified lymphocyte (LY) or alveolar macrophage (AM) subpopulations, or identified chemotactic and activating factors. So we have a better impression of interactions between LY, PMN, AM, and connective tissue cells (e.g. fibroblasts) today.
