Abstract

The endocrinology laboratory participating in an in vitro fertilization-embryo transfer (IVF-ET) program must provide timely and accurate data throughout the period of follicular recruitment. In addition, the laboratory may also be responsible for confirming pregnancy and contributing to the understanding of luteal phase function during successful and unsuccessful treatment cycles. The correct interpretation of daily hormone assay results will enhance the chance of obtaining a pregnancy during any given cycle since these results directly influence the timing and amount of medication provided and, therefore, the length of the “follicular” phase. Accurate timing for the administration of human chorionic gonadotropin (hCG) which induces completion of oocyte maturation can be assisted by measurement of gonadotropins, primarily luteinizing hormone (LH), during the latter part of the recruitment portion of the treatment cycle. Even though distinct and different problems may be encountered during the measurement of steroid and protein hormones, certain techniques may be applied to all hormone assays to increase validity and clinical relevance.
