Abstract
In membranous nephropathy patients with nephrotic range proteinuria have a poor prognosis (1, 2), while those with asymptomatic proteinuria show a more benigne course (1, 3, 4). Moreover, it has been reported (5) that patients with focal segmental glomerulosclerosis and massive proteinuria reached end stage renal failure in the first 6 years after renal biopsy. Since a positive correlation between the amount of proteinuria and the progression of renal damage emerges from these clinical studies and from experimental data (6), any treatment aimed at decreasing the severity of proteinuria may have a protective role on renal function. Thus, we have wanted to check the efficacy of captopril in 14 patients with glomerulonephritis and persistent proteinuria, in accordance with some recent clinical observations (7,8) that show the antiproteinuric effect of the angiotensin converting enzyme inhibitors (ACE-I).
