Streptococcus pyogenes, the group A streptococcus (GAS), is a common human pathogen that causes a wide array of infections, ranging from uncomplicated pharyngitis and pyoderma to invasive infections including sepsis, pyomyositis and fasciitis, pneumonia and toxic shock syndrome. Two nonsuppurative sequelae, acute rheumatic fever (ARF) and acute glomerulonephritis (AGN), are the most significant health problems stemming from GAS infection worldwide and account in large part for the intense clinical interest in this pathogen over the years. In the United States and other developed countries, where the incidence and severity of these diseases had declined over much of the 20th century,15,19 there has been a remarkable resurgence in the number of ARF cases over the last 10 years.60,69,70 In addition, a new clinical entity, streptococcal toxic shock syndrome has been described.58
