Risk Factors for Coronary and Cerebral Atherosclerosis in the Oslo Study

Abstract

The geographic differences in the extent of atherosclerotic lesions suggest that environmental factors are of importance in development of atherosclerosis (McGill 1968). The large variation in the degree of lesions among persons within the same population probably reflects individual susceptibility as well as differences in exposure to risk factors (Tejada et al. 1968). Since coronary atherosclerosis is necessary for the development of coronary heart disease (CHD), the assumption has been made that the risk factors for CHD are identical with those of coronary atherosclerosis. This may be true for some but not necessarily for all factors. Risk factors for CHD may lead to atherosclerotic lesions or to complication of lesions which cause occlusion of vessels. However, risk factors may also act directly on the myocardium and its conductive system, and thus contribute to the development of clinical disease. Since quantitative estimates of atherosclerosis are difficult to obtain in living persons, most information about risk factors have been obtained from retrospective studies on autopsy materials (Strong et al. 1978). More recently a few prospective studies have been initiated with careful documentation of certain risk factors during life, and standardized evaluation of atherosclerotic lesions at autopsy (Solberg et al. 1977; Sternby 1977; Garcia-Palmieri et al. 1977; Stemmermann et al. 1977). These studies show significant relationship between the levels of serum cholesterol, blood pressure and the extent of coronary atherosclerosis. The findings concerning the effect of smoking have so far been more uncertain. In this report we have included more cases from the Oslo Study than in a previous report (Solberg et al. 1977). Additional risk factors have been analyzed. The findings in the cerebral arteries are also included.
