Abstract
This chapter is an appendix of sorts to Chapter 3 and contains many tables reproduced, in part, from computer displays. These tables represent the intermediate analysis of the approximately 5000 individual case reports logged and coded in Appendix 2. The tables are given, with minor changes for clarity, as presented by the computer display (described in the previous chapter), and they follow standard statistical and computer science formats and language. These tables are reproduced in a separate chapter both as a reference for Chapter 3 and as a method of permitting an easier reading of Chapter 3.
