Management of Patellofemoral Arthritis

Abstract

Patellofemoral arthritis is most often associated with tibiofemoral arthritis.1 Surgical treatment of these patients with tricompartmental replacement has proven to be successful.2–8 Isolated patellofemoral arthritis is much less common. A large retrospective review of patients with idiopathic osteoarthritis showed only 3.8% to have isolated patellofemoral arthritis.9 These patients were predominantly elderly women with bilateral disease in a majority of cases. Surgical treatment of these patients is more difficult and less reliable than the long-term clinical success reported with total knee arthroplasty in cases of tricompartmental disease. The problem is even more difficult in the young patient for which tricompartmental replacement is to be avoided when possible. Multiple procedures have been described to treat isolated patellofemoral arthrosis, ranging from arthroscopic debridement to patellar arthroplasty.
