Abstract

In adults vertebral osteomyelitis differs from osteomyelitis at other sites in that, aside from postoperative infections, the infection almost always occurs by a hematogenous route. The tendency of osseous infections to localize in the vertebral body may be related to the nature of the vascular supply and the fact that the vertebral body is composed almost entirely of cancellous or spongy bone, which might provide a more suitable environment for infection to take place. Although it is difficult to estimate the relative proportion of vertebral to nonvertebral hematogenous osteomyelitis in adults, it appears that vertebral osteomyelitis is the more common disease (106). The typical patient is a middle-aged or elderly male without a history of underlying back disease and, in a significant number of patients, no history to account for a bacteremic episode.
