Linear Accelerator Radiosurgery of Cerebral Arteriovenous Malformations in the Pediatric Age Group

Abstract

In Leksell’s definition,radiosurgery means the destruction of intracranial targets, defined by a stereotactic procedure, by focusing radiation energy from external sources.1 In this new surgical method, the radiation beam was thought to replace the surgeon’s instrument in “open” procedures, mainly in the field of functional neurosurgery (Parkinsonism, intractable pain, psychiatric disorders), with the aim of avoiding the otherwise rare operative complications such as hemorrhage or infections. The first apparatuses employed in clinical practice were based on the principle of arc irradiation. After the very early operations performed either with the aid of particulated beams or low-energy X-ray beams,2 Leksell proposed the first multicobalt radiosurgical unit (the gamma unit)1,3 with the aim of giving nothing more than another surgical instrument to be manipulated by the neurosurgeon’s hand. In this apparatus, from 179 to 201 shielded 60Co sources, arrayed on a spherical sector (70 to 110 x 140°), the collimated beams are directed toward the target point, localized by the stereotactic procedure.
