Abstract
Recent observations in patients with myelopathy associated with spinal durai arteriovenous (AV) fistulas, which drain intrathecally into the veins of the spinal cord, indicate that venous hypertension can produce myelopathy.1–8 This clinical circumstance proves not only that venous congestion alone can produce myelopathy, but also indicates the type of clinical syndrome and the type of pathological injury that occurs to the cord with venous stagnation. It also calls for reevaluation of venous congestion as a more common mechanism of early and reversible cord injury in other conditions than has been generally recognized.
