Abstract

Lesions of the gastric wall and the gastric mucosa usually require removal either because of symptoms or for diagnosis. Wedge resections are sometimes possible for small lesions on the anterior wall of the body of the stomach; however, a careful excision via gastrotomy or partial gastrectomy have been required for lesions on the posterior gastric wall and those close to either the pylorus or the gastroesophageal junction. For a trained laparoscopic surgeon, a laparoscopic wedge resection of an anterior gastric wall lesion can usually be accomplished. Laparoscopic endogastric surgery has been gaining popularity. Endogastric surgery was first described by Way et al. using special trocars for intraluminal surgery to do pancreatic cystgastrostomy.1 Several reports have been published subsequently that use similar techniques to remove gastric lesions.2–8 The method has been given several different names, such as endoluminal surgery, endogastric surgery, laparoscopic intragastric surgery (LIGS), and transgastric surgery. The techniques vary slightly, but along with a gastroscope all involve a laparoscopic approach with special trocars passed through the abdominal wall and the anterior gastric wall into the stomach.
