Abstract

Total proctocolectomy with permanent end-ileostomy is indicated for those patients who require surgical treatment of ulcerative colitis, yet are not candidates for restorative proctocolectomy with ileoanal anastomosis. For the most part this includes patients with preoperative anal incontinence, elderly patients, and those patients with rectal adenocarcinomas. In addition, a small number of patients will prefer to have total proctocolectomy with permanent end ileostomy over an ileoanal anastomosis. Concerns over functional results, risk of pouchitis, and need for multiple surgeries are commonly sighted reasons for patients to reject ileoanal anastomosis in favor of total proctocolectomy with permanent ileostomy (1).
