Diagnosis and Medical Management of Recurrent and Chronic Sinusitis in Adults

Abstract

Diagnosis of sinusitis is based on a history of chronic symptoms suggesting inflammation arising from the paranasal sinuses (purulent nasal secretion, postnasal drainage, facial pain, nasal obstruction) combined with a positive imaging test inferring the location of the pathology. Medical management of recurrent and chronic sinusitis is based on empiric goals. Adjunctive agents are frequently prescribed in addition to antibiotics to aid drainage of retained secretions through the sinus ostia into the nasal cavity. Scientific data to support combination therapy are based on clinical experience. It is unclear how many cases of chronic sinusitis result from incomplete treatment of acute sinusitis.
