Difficult Decisions in Thoracic Surgery

Abstract

In contrast to other surgical subspecialties, robotics has very slowly made its way into the world of thoracic surgery. One of the arguments slowing down the widespread use of robotics in our field is a lack of evidence that the robotic platform is better than the more seasoned video-assisted thoracoscopic approach to the diseases we treat. Considering the role of robotic lobectomy in the treatment of early stage non-small cell lung cancer (NSCLC), the current level of evidence supports a conditional recommendation for its safety, its effectiveness and oncologic outcomes. However, at this time, when comparing to video assisted thoracic surgery (VATS) lobectomy, the cost to benefit ratio does not favor robotic lobectomy.
