Abstract

Acute type A aortic dissection (AADA) is a potentially highly dangerous cardiovascular condition requiring emergency surgery. If I would suffer symptoms that could be caused by AADA, I would ask for immediate transfer in a tertiary cardiovascular center with extensive experience with acute and chronic aortic surgery, that is capable to offer all surgical and endovascular options 24/365. I would expect immediate surgery following diagnosis and that any additional complication (especially malperfusion) would be recognized and treated adequately. Furthermore, surgical treatment should be extensive enough to prevent as much as possible potential mid- and long-term complications such as aortic valve regurgitation and dissection-related complications in the proximal and downstream aorta.
