Preoperative Cardiac Evaluation Before Noncardiac Surgery

Abstract

Over the past 30 years, there has been a great deal of interest in defining the optimal preoperative cardiac evaluation before noncardiac surgery. From an evidence-based perspective, the studies transition from cohort studies involving risk assessment to cohort studies of interventions and finally to randomized controlled trials. Since 1996, there have been a series of evidence-based guidelines published from a diverse group of societies including the American Heart Association/American College of Cardiology Foundation, the American College of Physicians, and the European Society of Cardiology. While the specific details of the recommendations differ slightly, the overall approach is very similar. This chapter will review the evidence that underlies these guidelines and place it within the context of the optimal cardiology consultation.
