Abstract
The Infiection Navigator system is a low-tech and low-cost innovation which helps empower a care coordination team to help patients receive a consistent and efficient workup for important healthcare issues. These health care related “Infiection Points” can range from a new lab finding (e.g. Hematuria or blood in the urine) to a new diagnosis (e.g. Atrial Fibrillation, Hypertension). A physician initiates the Infiection Navigator process by choosing a “Pathway” message in their electronic medical record (EMR) system (See Fig. 3.1) and sending it to their care coordination team. These care coordinators, or Infiection Navigators, then notify patients about what to expect in the coming weeks, provide them with educational materials, obtain referral authorizations and help them set up tests and specialty visits in the correct sequence and timeframe.
