Abstract
Pulmonary hypertension may persist after MitraClip repair of mitral regurgitation. The iatrogenic atrial septal defect (ASD) created by the MitraClip guide may result in significant left-to-right shunting and aggravate the pulmonary hypertension. In this case example, a patient with persistent pulmonary hypertension and severe left-to-right shunting from the iatrogenic ASD was treated with an atrial septal occluder at the end of the MitraClip procedure.
