Abstract

The transcochlear approach to the skull base evolved out of the need for a transtemporal approach to lesions medial to the porus and anterior to the brainstem. The translabyrinthine route interposed the facial nerve between the surgeon and anterior temporal bone lesions. The suboccipital approach placed the cerebellum and brainstem between the operator and the tumor. Rerouting of the facial nerve allowed for total tumor removal with preservation of facial nerve function.
