Analysis of UK Offshore Accidents & Incidents

Accident and incident data for the UK offshore industry is collected via two reporting forms: Offshore Incident Report (OIR) forms OIR/9A and OIR12. OIR/9A deals with all offshore accidents and incidents, and OIR12 records details of “loss of containment” incidents, introduced specifically to cover recommendation 39 of the Cullen Report of the Piper Alpha disaster of June 1988. A loss of containment incident recorded on an OIR/9A triggers a complementary OIR12, which requests further technical details of the event.
