Abstract

The full, detailed and objective investigation of any accident is essential for three reasons:

  1. 1)

    To establish the facts of the case to dispel suspicion and speculation;

  2. 2)

    To enable changes in procedure and equipment to be made to prevent a future recurrence;

  3. 3)

    To establish the cause of the accident with accuracy so that proper and appropriate compensation can be determined.
