Abstract
Pleural and pulmonary complications are common in the connective tissue disorders. Estimates of incidence depend on the methods used to detect them and, in patients with rheumatoid arthritis for example, have ranged from 2% when chest radiographs alone were used to 45% when pulmonary function tests were included. The main management decisions centre on the use of corticosteroids. These are very effective in suppressing some of the pleural and pulmonary complications but completely ineffective in others. The decision to use corticosteroids for specific respiratory problems must consider the effect of steroids on other aspects of these multisystem diseases. Steroid trials need care since, if steroid withdrawal is too abrupt, the underlying disease may flare up.
