Abstract
Trauma is the leading cause of death in the under 44 year age group. Although the goal should be prevention, when such injuries occur prompt recognition and proper management based on sound judgment and experience are critical in limiting complications including potential lifelong disability. Iatrogenic injuries comprise a distinct subgroup of trauma. During surgery, it is important that we strive to minimize such preventable occurrences by maintaining a thorough understanding of the operative anatomy, ensuring meticulous tissue dissection and obtaining intraoperative consultations from other surgical disciplines, when necessary. During the past two decades the management of urologic injuries has evolved from a predominantly surgical approach to one of close observation with limited intervention using minimally invasive techniques. This paradigm change is clearly evident with renal injuries, most of which are managed non-operatively; intervention, when necessary, is accomplished usually with endovascular and endoscopic techniques (e.g. selective arteriography, stent placement). Similarly, select lower urinary tract and genital injuries can be managed conservatively. A thorough understanding of the mechanisms of genitourinary trauma and their proper management is paramount in improving preventive measures and optimizing treatment outcomes.
