Abstract
Chalmers produced a comprehensive recent review on nerve compression syndromes in the upper limb, including anatomy, clinical features, diagnostic tests and treatment. The large number of systemic and local conditions contributory to a carpal tunnel syndrome were tabled, with references to each. The clinical presentation and treatment of the pronator teres and anterior interosseous nerve syndromes were discussed. The causes of ulnar nerve compression both at the elbow and in relation to Guyon’s canal were tabled. The arguments were marshalled for simple decompression as against anterior transposition for release of the ulnar nerve compression at the elbow. Subcutaneous and intramuscular beds are considered unsatisfactory and transposition deep to the flexor muscles is recommended. Howard considered controversies in relation to the five tunnels in the forearm and wrist where nerve entrapment may occur. A positive EMG report reinforces a clinical impression but if negative, does not exclude an entrapment syndrome.
