Contraindications to regional analgesia in labour?

Abstract

The traditional contraindications to any form of local anaesthesia that involves plunging a needle into unknown territory are coagulopathy and sepsis, although these are not the only risk factors for spinal haematoma and abscess. For neuraxial blockade in obstetrics, other contraindications might include circulatory disturbance and shortage of staff trained to apply the block or care for the parturient. None of these is absolute and all require some qualification. Many would add lack of consent to this list, but such a constraint applies to any treatment that is offered.
