Abstract

Bassini’s principle of the importance of the transversalis fascia in repair of inguinal hernias was incorporated into the influential Shouldice repair. Later, Usher’s introduction of monofilament polypropylene mesh and Lichtenstein ’s concept of tension-free repair revolutionized the methods used today to achieve reliable hernia repairs with very few recurrences. Laparoscopic surgeons have followed Stoppa and Nyhus in using preperitoneal placement of mesh. Over the past 10 years, the surgeons at the National Ambulatory Hernia Institute have incorporated these ideas into an open, transinguinal preperitoneal placement of polypropylene mesh under minimum tension, with an excellent outcome.
