Operative Hip Arthroscopy

Abstract

The term femoroacetabular impingement (FAI) was first described by Ganz et al. as an anatomic abnormality whereby there is a loss of femoral head-neck offset and prominence at the head-neck junction. The pathomechanics of hip impingement has been implicated as a significant cause of hip pain, and there is evidence to suggest that these changes are associated with the premature development of hip osteoarthritis. In addition, focal rim impingement of the acetabular labrum with subsequent damage and tearing has been suggested as an important step in the pathway of hip degeneration. Recently, arthroscopic surgical techniques have evolved to treat a spectrum of pathologic conditions associated with femoroacetabular impingement. Despite these advances, however, hip arthroscopy remains a technically challenging procedure with a steep learning curve. As a result, computer-assisted navigation and modeling have emerged as a potential solution to improve both the preoperative planning for FAI lesions, including determination of the location and size of cam and rim lesions, as well as increasing the accuracy of intraoperative correction of the osseous deformity. The goal of these technologies is to improve efficiency, reproducibility, and long-term clinical outcomes by minimizing the potential for incomplete correction of the osseous deformities.
