Oral Therapy for Bladder Pain Syndrome Directed at the Bladder

Abstract

Initial oral therapy for BPS can include amitriptyline, cimetidine, hydroxyzine, and/or pentosan polysulfate sodium. Cyclosporine should be reserved for use by experienced clinicians after less potentially morbid alternatives have been exhausted. Few of the oral therapies commonly used for the treatment of BPS have unequivocal evidence of efficacy in large, multicenter, randomized controlled clinical trials. There is little evidence that any change the natural history of the disease, though many seem to be effective in relieving symptoms in individual patients. Tachyphylaxis may be a problem.
