Abstract

Case management has been defined as a process where a single person takes responsibility for maintaining a long-term supportive relationship with a patient, regardless of where the patient is and regardless of the number of agencies involved (Intagliata, 1982). Its function is to assist patients to identify, secure and sustain the range of internal and external resources they need to live as independent a life as possible in the community (Rapp and Kisthardt, 1991). There are similarities with the concept of a ‘key worker’ as described in Building Bridges (Department of Health (DoH), 1995) and at times the two terms appear interchangeable. This method of service delivery has proved to be particularly important in the configuration of services for those patients with severe and enduring mental health problems such as schizophrenia.
