Any organization needs to be reviewed periodically to see that it is still appropriate for current needs. This is true both of the organization as a whole and of its individual parts. When doing so it is useful to remember some of the common mistakes that can be made setting up organizations, that can develop with time or that can develop in rapidly changing conditions. This chapter is intended as a brief guide to these mistakes. The questions can be used as a checklist to help expose particular mistakes.
