Abstract
1. The physical health and longevity of persons in the western world has improved dramatically during the course of the last 100 years. In the 1850s the average Englishman had a life expectancy at birth of 40 years: today this is nearer 70. In 1878 the infant mortality in York was reported by Rowntree to be 247 per 1,000 births in the poorer classes and 94 per 1,000 live births among the well-to-do; in England at the present time the infant mortality is on average well below 20 per 1,000 live births. However, it is interesting that although the overall birth mortality rate in Britain is low there still remains a class gradient so that the mortality rate in higher social classes is less than that in the lower classes. New diseases are appearing from time to time and the current new disease is AIDS, which is now reaching epidemic proportions. At present there is no cure but the impact of AIDS is undoubtedly going to change health care provision and use many resources over the next ten years or so.
