Abstract

A study has been performed on common cause failures (CCF) of safety and relief valves (SRV). The analyzed data set of 149 CCF events was derived from the International CCF Data Exchange (ICDE) database, to which several countries have submitted CCF event data. This paper gives an overview of the results and presents several insights about this in-depth review of the SRV events.
