A 72-year-old man was found to have an enlarged cervical lymph node in a routine physical checkup. He denied having fever, night sweats and weight loss. Physical examination showed no hepatosplenomegaly. CT scan revealed no deep-seated lymphadenopathy. Peripheral blood examination demonstrated a total leukocyte count of 4,500/μl with 72% neutrophils, 23% lymphocytes, 4% monocytes, and 1% eosinophils. His hematocrit was 40%, hemoglobin 13 g/dl, and platelets 152,000/μl. A bone marrow aspirate showed 9% lymphocytes and the core biopsy revealed no lymphoid infiltrate. A lymph node biopsy was performed (Figs. 28.1 and 28.2).
