Building Community Collaborations Around Care for the Uninsured

The local hospital convened a group of community stakeholders as part of their strategic planning process to address access to healthcare for the estimated one million uninsured in the county. The participants included area hospitals, the county health department, healthcare professionals, community health centers, local governmental officials, business owners, legislators, local advocacy groups and many health and human services organizations including the food bank, women’s shelter, and job advocacy groups. Their day-long process consisted of an update on current reality in terms of the numbers of uninsured, the demographics, utilization of the local healthcare system, economic impact, and health status. Community readiness was assessed as well as assets identified for use in future work. Logic models were developed, which identified a lack of coordinated care for the uninsured, the lack of a collaborative health information system, and a need for services in two specific areas of the community. Over the next 24 months, the community collaborative initiated a shared information system for the uninsured to track healthcare service utilization and the care received, recruited volunteer physicians and nurse practitioners to provide primary care services at two health-department-sponsored locations, and began the process of case managing uninsured individuals with chronic diseases. The collaborative continues to meet every month to monitor progress, add additional community partners, and seek long-term sustainable systems to care for the uninsured in their community.
