Laparoscopic adrenalectomy has rapidly superseded open adrenalectomy for most cases of benign adrenal disease. Conversely, open adrenalectomy is preferred when adrenocortical carcinoma is the diagnosis. When the diagnosis is uncertain, but definitely includes adrenocortical cancer, the decision-making process becomes more complex. Chapter 35 discussed options for management of the incidentaloma with benign features. In this chapter, an incidentaloma with some concerning features is presented, and the pros and cons of open versus laparoscopic adrenalectomy are debated.
