JB is a 23-year-old Mexican man who fell into a metal pipe at work and struck his left eye and immediately noted loss of vision with severe pain. He was taken to the emergency room and was diagnosed as having a ruptured globe. An ophthalmology resident at the government hospital did a primary closure the next day. He told the patient he had a cataract and would need surgery the following week. One of the staff ophthalmologists in the residency program had an interest in echography and had recently taken a course in anterior segment techniques and offered to examine the patient before surgery.
