Abstract
Most episodes of hospitalisation will incorporate some form of discharge planning in the quest for enhancing clinical practice and facilitating satisfactory transitional care for the child and family. (Smith, 1995). The outcome of discharge planning can have irrevocable effects on both the child and family, so it is essential to get it right. There has been much discussion regarding how and when this should happen, but is it rhetoric or reality?
